A Registered Dietitian uses their clinical skills and assessment tools to perform a thorough assessment of each client. They then create appropriate goals in conjunction with the Home and Community Care Support Services (HCCSS) to treat the client. As a whole, Dietitians provide professional care to clients in order to help clients reach their goals.

Use of your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the HCCSS referral and other sources
Function as a member of a therapy team who are accountable for the therapy care it provides
Develop appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the HCCSS goal for treatment
Committed to quality and accuracy of care
Provide Professional Clinical therapy services (disc. specific) for clients in the community, fostering client-centered care and consistent with the Standards of respective Colleges.
Provide Assessment, Intervention and Discharge Planning, within the Scope of Practice, utilizing standard and non-standard assessments and interventions which are reflective of Best Practice.
Provide appropriate documentation according to Professional Standards and HCCSS contract guidelines.
Communicates with HCCSS, according to its standards
Collaborates with other health care team members and participates in multi-disciplinary team/family conferences, as required.
Participates in Quality Management and Risk Management programs/activities and in other projects/committees and student preceptorship, as needed.
Demonstrates a clear understanding of Community Resources, including funding.
Must have a degree from an accredited undergraduate program
Current, unrestricted license to practice with College of Dietitians of Ontario
Proven computer skills, including e-mail, word processor and work scheduling systems
Must be fluent in English and able to communicate effectively in both oral and written form
Experience in providing service in a community care setting is preferred.
Must have a reliable vehicle and driver’s license
